Home Care Services
There are two types of home health care,
Skilled Home Health Care and Private Duty Home Care.
The goal of skilled home care is to assist the patient and their caregiver to be
as independent as possible in a home setting. This service does require a physician order.

Skilled Home Health Care allows individuals who have been hospitalized, disabled, or homebound, recovering from an illness and/or injury an opportunity to recuperate in the privacy of their own home. Under the direction of the physician of your choice, this agency can provide a combination of services including Skilled Nursing, Physical, Occupational, or Speech Therapy, Medical Social Services, Registered Dietician/ Nutrition Services, Home Health Aides.
Skilled Home Health Care is covered by insurance, oftentimes Medicare. Skilled home care provides services by Registered Nurses, Medical Social Workers, Home Health Aides, Physical, Occupational, and Speech Therapists.
Private Duty Home Care is for individuals who want to remain in the comfort of their home, yet need assistance with daily living chores and/or personal care assistance. These services can eliminate unnecessary hospitalizations and facilitate a safe environment for your loved one. A personal caregiver can come to your home to provide the necessary support that will foster independence and provide family members reassurance that your physical, social, spiritual, emotional, and recreational needs are being met.
Private Duty Home Care is primarily paid for by the patient receiving the services, family member, or trustee. Some people have long-term care policies or another type of insurance that would cover private home care.

Home Care Assistance offers two types of services, live-in and hourly care. Live-In Care provides older adults who prefer the comfort and safety of round-the-clock monitoring and care. A caregiver comes to your home and stays around the clock, ready to assist with activities of daily living and provide companionship.
Hourly Home Care allows clients to use our caregivers on an hourly or as-needed basis by helping with daily activities during certain times of the day, night, or weekends.

Lutheran Social Services of Michigan
Lutheran Social Services of Michigan Home Care solutions offer independence and a sense of connection, allowing people to remain in their natural environments for as long as possible – perhaps throughout the duration of their lives.
We help senior adults and others who need temporary or ongoing help to live independently. We provide services to those who need a little help to remain in their homes. Our services include:- medication assistance

Lutheran Social Services of Michigan offers in-home care for seniors on Medicaid. Caregivers provide support in areas that include assistance with bathing, grooming, dressing, transportation, errands, light housekeeping, meal preparation, medication reminders, or just someone to offer companionship. To maintain affordability for every senior, this program does not provide medical services to ensure that the care you receive is covered under the home care guidelines.

Private Pay Home Care Assistance to help you or a loved one maintain your independence at whatever stage you find yourself, the Home Care Assistance program brings trained professionals right to your home to deliver support. Choose from non-medical services that include assistance with bathing, grooming, dressing, transportation, errands, light housekeeping, meal preparation, medication reminders, or just someone to offer companionship. This program is recommended for seniors who do not require medical services but who are looking for support and assistance.

Monroe County DHHS Assistance is available if you need help with daily activities and meet eligibility requirements. To receive Home Help services, a person must be eligible for Medicaid and require physical assistance with at least one activity of daily living. An assessment will be provided to determine whether or not you need assistance with daily care. If you or your loved one needs help, contact the Adult Services Unit at your local county Department of Health and Human Services. |
